1. To qualify, patients must be in good overall oral health, deemed by Dr. Vincent M. Guido DDS.

2. WOW! club member benefits include two complimentary whitening treatments limited to Sinsational Smile 20 Minute Whitening Treatment only and 10% discount on fees for any X-rays. Custom trays and whitening syringes are not included. Only what is included in the Sinsational Smile Patient kit applies to WOW! club members.
\n\n\n\n
3. New patients wishing to become a WOW! club member must first receive a Comprehensive Exam and necessary X-rays (Panoramic X-ray, Bitewing X-rays, and a Full Mouth Series) before the patient qualifies for WOW! Club membership.
\n\n\n\n
4. Per recommended frequency by Sinsational Smile, whitening treatment will be one every six months to coincide with biannual hygiene therapy.
\n\n\n\n
5. Complimentary whitening treatment must be scheduled on same day as hygiene therapy unless approved by Guido Family Dentistry. If appointment is canceled within 24 hours of appointment time or the member fails to show up for scheduled appointment, that one complimentary whitening treatment will be forfeited.
\n\n\n\n
6. Guido Family Dentistry has the right to reject applicable whitening treatment for any reason, specifically due to potential benefits and risks based on current state of oral health.
\n\n\n\n
7. Qualified X-ray procedures include 2-4 Bitewings, Panoramic, Full Mouth Series, and periapical (PA).
\n\n\n\n
8. WOW! club membership is valid for one year from the date membership fees ($150) are paid in full. No benefits will be offered until fees are paid in full.
\n\n\n\n
9. Membership benefits are not transferable to any other patient or family members, current or new; only valid for WOW! club members.
\n\n\n\n
10. Evidence of fraudulence, including but not limited to changing the name of the member on the membership card to either a current or non-member, changing the expiration date, and attempting to receive more than TWO complimentary whitening treatments, without permission from Guido Family Dentistry will result in forfeiture of WOW! club membership for one year and all paid fees and benefits exclusive to WOW! club membership.
