\n\nWe understand that injuries and complex medical conditions can be a significant emotional and financial burden, therefore as an approved service provider for WorkCover and the Transport Accident Condition (TAC) insurance claims, we offer ‘no gap’ rehabilitation care for all patients. Rise Health Group also accepts the Medicare benefit as full payment for services provided as a part of the Chronic Disease Management program and for all patients with a current Gold/White DVA Health Card.

Medicare

\n\nMedicare offers The Chronic Disease Management (CDM) program, formally Enhanced Primary Care or EPC, which assists in the management of patients with 'chronic and complex' medical conditions. These plans come from your GP and entitles you to a rebate of $52.95 per sessions, with a maximum of 5 sessions per calendar year. At Rise Health Group these plans are accepted by our Podiatrist, Physiotherapists and Exercise Physiologist.

CDM plans are accepted at Rise Health Group however, all patients must have a GP Management Plan and Team Care Arrangements prepared by their GP as a part of their referral.

Transport Accident Commission (TAC)

As specialists in the area of exercise for rehabilitation our practitioners not only have a sound understanding of how factors such as age and lifestyle impact on a patients recovery but they will also provide ongoing support through each phase of your rehabilitation program and get you moving and returning to your daily activities as soon as possible.
